What happens in the ninety minutes?
Assessment before recommendation — that’s the whole structure. Two hours are held in the diary for it, and most appointments use about ninety of those minutes. The spare half hour is not padding — it is there so that a consultation which needs longer can simply take longer, without the next person waiting or yours being cut short.
The appointment is ninety minutes because that’s what a proper read of your skin takes, not fifteen minutes and a menu.

Why isn’t the fee credited toward treatment?
Because that’s deliberate. The $150 pays for the assessment and the written plan — they are the product, not a pathway to one. The moment a consultation fee converts into treatment credit, the appointment has a sales target. This one doesn’t.
It keeps the advice clean. Nothing about the ninety minutes depends on selling you a treatment, so the plan can say wait, or not yet, or nothing — and you can take it home, to another clinic, or nowhere at all. Leaving without booking anything is a normal outcome here.
