These change what we can do, or how we space it. Most of them do not stop anything — they change the plan, which is usually the point. And nothing you tell us here costs you your booking fee: if something means waiting, the fee moves to the new date, or comes back if you ask for it.
The doctor needs this before writing anything, and it changes how skin heals. The ones people leave out are the ones taken occasionally, and the ones nobody thinks of as medication — so they are asked for by name below.

Including things that are not strictly allergies. If something has ever reacted badly, we want to know — a reaction is as useful to us as a diagnosis.
This is the part that changes what happens on the day, and it is the one people most often skip. None of it is a test — there is no wrong answer about your own routine.
This part is short, and it is not consent to any treatment. What you agree to on the day is a separate conversation with your nurse, who first tells you what is involved and what it will not do.
What you can change afterwards, and what you cannot. You can stop your script at any time, which also stops us contacting you about your care. You can turn off anything optional above whenever you like. What cannot be undone is the record itself — we have to keep it for seven years after your last visit, and that is the law rather than our policy. You can always ask to see it, and you can always have a correction recorded against it.
Your signed copy is kept in your record, and you can ask us for it any time. If something here turns out to be wrong, tell us — we do not edit it, we record the correction alongside it, so the record always shows what was known when.
You have filled this in before, so this is the short version — five questions about what has moved since you were last here, and four that are asked every visit. If nothing has moved, it takes a minute and your nurse still knows that for certain, which is the point of asking.
